Contact information for your child's character reference (children's/youth pastor, coach, former teacher, principal, etc.). Please let this person know in advance of filling in this application. 2. The most recent report card, if applicable 3. A psychoeducational report, if applicable 4. Individualized educational plan documents, if applicable 5. Online payment (credit card) information ready for the $250 registration fee 6.
